Criminal Records Bureau (CRB) Form

The CRB helps organisations to perform better by screening out candidates who may be unsuitable for certain kinds of work. It is a government initiative and provides help to organisations in England and Wales to make more informed recruitment decisions.

You may be asked to complete a DISCLOSURE APPLICATION FORM, which will contain certain questions. You will be required to answer these questions in order to progress your application.

Examples of the types of questions you will be asked are as follows:

1. Do you have any unspent criminal convictions?
2. Consent of Applicant – You will need to consent to the CRB checking the details you have provided in support of your application against data sources specified in the notes for guidance, in order to verify your identity and process your application. These details may be recorded and used to assist other organisations for identity verification purposes.
3. Declaration – You will need to confirm that the information you have provided is complete and true and understand that knowingly to make a false statement for this purpose is a criminal offence.
